During a 2015 broadcast on Fox 5 San Diego, a significant mishap occurred when anchor Kathleen Bade was reporting that the suspect in a legal case would not face charges. Unexpectedly, an image of then-President Obama appeared on screen, which was intended for a different story. This image lingered on the screen for approximately five seconds, giving viewers a misleading impression.
The newsroom staff promptly noticed the error, but the misleading image had already been broadcast. Recognizing the severity of the mistake, Fox 5 San Diego took responsibility and issued a formal apology to its viewers two nights later.
